If you have received a prompt in your account to submit your W-9 form, it means that you have already received $600 or more in payments from Dynata this year, by redeeming points, dollars, or otherwise from Dynata’s survey panels (e-Rewards, e-Rewards Medical, iPoll, Valued Opinions, QuickThoughts, OpinionWorld, Opinion Outpost, Expert Opinions, Vindale, OneOpinion, Zap Surveys), or by participating in focus groups hosted by Dynata.
A W-9 form request will be emailed to your registered e-mail address. Please follow the instructions in the email to complete the form.
